i3Health has developed a dual part education activity for pharmacists on Multiple myeloma with special guest Joseph A. Kalis, PharmD, BCOP and Ambulatory Oncology Pharmacy Specialist.  Multiple myeloma is a clonal plasma cell malignancy characterized by several cytogenetic alterations. Treatment planning for individual patients is complicated by factors such as age, frailty, comorbidities, eligibility for autologous stem cell transplantation, and risk stratification.  Clinicians need to continually update their working knowledge of emerging data and consensus recommendations that can rationally inform clinical decision making. In Module 1 of this continuing pharmacy education (CPE)–approved activity, Joseph A. Kalis, PharmD, BCOP, Ambulatory Oncology Clinical Pharmacy Specialist at the University of Colorado Health, will explore evolving treatment paradigms in newly diagnosed multiple myeloma, including the efficacy and safety of anti-CD38 monoclonal antibodies and supportive care considerations for their use.

The Human Cost of Ignoring DPD Deficiency | PGX For Pharmacists

DYPD is a gene that provides instructions for making an enzyme called dihydropyrimidine dehydrogenase (DPD). This enzyme plays very important in breaking down a substance called uracil, which is found in our DNA and RNA. Mutations in the DYPD gene can lead to decreased or absent DPD activity, which if you think about it can cause a buildup of uracil in the body.  If someone is DPD deficiency it can make it difficult for the body to process certain chemotherapy drugs and can lead to severe side effects, such as nerve damage, GI problems, and even death. Genetic testing before being placed on a chemo agent, to see if there is a DPD deficiency is vital.  According to Advocates for Universal DPD/DPYD Testing (AUDT), Europe is becoming the standard of care for testing for this gene and only in some selected institutions in the US the testing it being done.  AUDT is made up of a group of patient advocates and medical professionals who are seeking to improve treatment outcomes for patients that are preparing to receive chemotherapy based on fluoropyrimidine drugs. One of the founders of this non-profit organization, AUDT, Karen Merritt is our guest today. She lost her mother in 2014 due to being DPD deficient and after getting her first infusion of 5FU. Karen dedicated herself to advocating and raising awareness about pre-testing for DPD deficiency before fluoropyrimidine chemotherapy administration to reduce unnecessary suffering. Teach, M.D., M.P.H., is the associate dean for Pediatric Faculty Affairs and chair of the department of Pediatrics at George Washington University School of Medicine and Health Sciences; and director and principal investigator of IMPACT DC (Improving Pediatric Asthma Care in the District of Columbia), a care, research, and advocacy program focused on under-resourced and largely minority children with asthma. Dr. Teach also serves as the Washington, D.C., site principal investigator for the NIH/NIAID-funded Inner City Asthma Consortium. Olufunke Sokan, Pharm.D., advanced practice pharmacist at the University of Maryland School of Pharmacy and lead author of the study to discuss the study findings and how clinicians used tech to monitor prescription fill patterns and counsel patients accordingly. Patients with chronic heart disease and lung disease are more likely to fill their prescriptions when enrolled in a Mobile Integrated Healthcare and Community Paramedicine (MIH-CP) program after hospital discharge according to a new study by the University of Maryland just published this month.   Prescription first-fill rates increased by nearly 20% for patients with congestive heart failure (CHF) and 25% for patients with chronic obstructive pulmonary disease (COPD) in the first 30 days after enrollment. The six-month pilot also improved medication adherence by 8% to 14% in the 60 days after a pharmacist-led intervention. Because the improvements did not continue beyond 30-60 days, the study authors suggest that adding additional interventions could achieve an ongoing impact on medication adherence. the press release!

Biography Dr. Bratberg is well acquainted with the challenges community pharmacists and pharmacies face regarding opioid safety, overdose, harm reduction, and addiction pharmacotherapy. In 2012, Dr. Bratberg, along with a URI student pharmacist, co-developed an overdose education and naloxone training program for pharmacists in the first-in-nation statewide Collaborative Pharmacy Practice Agreement for naloxone.   He is an unpaid consultant for prescribetoprevent.org and prevent-protect.org, websites devoted to opioid overdose education and naloxone training for health professionals, community members, and health departments. In 2015, he was selected to serve as a member of the RI Governor’s Overdose Prevention and Intervention Task Force, to create and implement a plan that addresses prevention, treatment, overdose reversal, and recovery of citizens affected by opioid use disorders. He has been a consultant or co-investigator on federal grants from NIDA, CDC, AHRQ, NIGMS, RI EOHHS, and the URI Foundation. Dr. Bratberg is the 2016 NASPA National Cardinal Health Generation Rx Award winner. The Generation Rx Champions Award honors a pharmacist who has demonstrated outstanding commitment to raising awareness of the dangers of prescription drug misuse among the general public, as well as the pharmacy community. Finally, Dr. Bratberg is the guest editor of the first ever Special Issue on Opioid Safety and Naloxone of the Journal of the American Pharmacists Association (Mar/April 2017). Ernest Dole Clinical Pharmacist  University of New Mexico Hospitals  Albuquerque, New Mexico  About:  Dr. Dole is Clinical Pharmacist at University of New Mexico Hospitals (UNMH), Department of Clinical Pharmacy/Pain Consultation and Treatment Center (PCTC); and Clinical Associate Professor at University of New Mexico Health Sciences College of Pharmacy. At UNMH PCTC, Dr. Dole manages ambulatory care patients with chronic pain, as well as participating on an inpatient Pain Service. He received his B.S. in Pharmacy from Colorado, with honors, and his Doctor of Pharmacy degree from Tennessee.  He completed an ASHP accredited Clinical Pharmacy Practice residency at Memphis VAMC, 1988. Dr. Dole was the ninth Pharmacist Clinician in New Mexico in 1996, was recognized as an ASHP fellow in 1997, passed the Board of Pharmaceutical Specialties Pharmacotherapy exam in 1997, and Certified Diabetes Educator exam in 2000. Dr. Dole has twice been nominated for the NMSHP Pharmacist of the Year. He has been the Chair of American Society of Health-System Pharmacists Section of Home, Ambulatory and Chronic Care Practitioners and has served as a member on ASHP Councils and Task Forces.  He has been president of the New Mexico Society of Health-System Pharmacists in 2008-2009. Dr. Dole received the Volunteer Faculty of the Year Award and Preceptor of the Year Award from University of New Mexico Health Sciences College of Pharmacy in 2009. In 2012, Dr. Dole received the Dorothy Dillon Memorial Lecture Award from the New Mexico Society of Health System Pharmacists. This is the organization’s highest honor. In 2013 Dr. Dole received the ASHP Section of Ambulatory Care Practitioners Distinguished Service Award. His research interests include the impact of culture on pharmaceutical care, substance abuse in the elderly, and obtaining federal recognition of pharmacists as healthcare providers.  His research interests with respect to chronic pain control include the effect of high opiate doses on testosterone levels, the effect of opiates on sleep apnea, and the use of non-opiate medications in the therapy of chronic non-cancer pain.
