#24: Drugs In my Pocket (and I don't’ know what to do with then)

#24: Drugs In my Pocket (and I don't’ know what to do with then)

This week we are talking about “drugs”, specifically the world of PBMs with Dr. Lee Strandberg. Pharmacy Benefit Managers(PBM)  are the middlemen between your pharmacist and your prescription and your insurance company. Yep.. more middlemen. If you have never heard of the term PBM, well you are not alone. Most people have no clue that they exist, how they work and how the cost or medications is controlled, negotiated or paid for. They are the one paying the pharmacist for your medication! While there are approximately 60 PBS, the 3 largest ones currently control 80% of the market. We discuss both arms of the  pharmacy world- how the pharmacy acquires the medications and purchases them. Hang on as we  unravel the mysterious chain of events on how the pharmacy gets paid by the PBM and how they negotiate the prices with your insurance company. We do get a little deep in the weeds on cost control and price negotiations, so pay attention. We only WISH that getting a Zpack  could be as simple as buying a head of lettuce from your local farmers market. Dr. Lee Strandberg is our guest this week who intimately knows the ins and outs of the complex layers of the pharmacy world. He is a Professor Emeritus, Pharmacy Economics and Public Health Oregon State University and was Director, Managed Care pharmacy for SHS Plans from 2005-2011. He was appointed by Oregon Governor Kitzhaber to the OR Health Policy Board, Medicare-Medicaid Integration of Care and Services work Group which wrote part of the CCO and reporting structure. He was a committee member on the OR Evidence Based Review of Prescription Drugs Preferred Drug List. During the past 40 years he has presented papers, seminars and consulted on the impact of pharmaceutical use on health care costs and quality. He has been a consultant to governmental agencies in Australia and Canada. He currently works as a Pharmacy Director for Kannact, Inc.Don’t forget to reach out to MR Insurance Consultants where their goal is to assist physicians in obtaining the most comprehensive coverage available to fit their unique situation. #22: “When the Oncologist gets Cancer” The Doctor as Patient with Dr. Reema Batra

#22: “When the Oncologist gets Cancer” The Doctor as Patient with Dr. Reema Batra

What if you became the patient of the very thing you fought? Hear this amazing story from Dr. Reema Batra, our guest this week in our 3rd installment in our series of “The Doctor as Patient”. Dr. Batra is a Board Certified Medical Oncologist and Hematologist. Her story is not only ironic but provides a unique perspective, having recently been diagnosed and treated for early stage breast cancer. She graciously shares the events that led to fortunate early diagnosis, her unique ability to advocate for her own treatment plan and how she dealt with the  emotional struggles. Now having experienced what her patients face, she knows firsthand their uncertainties and fears. Literally walking  in their shoes has provided a shared bond and ability to offer unique support for them through their journey. More about Dr Batra: She has been practicing since 2007. Initially she worked in the academic setting at NYU focusing on cancer care and healthcare disparities. She led a project to start a Breast Care Center at Woodhull Medical Center in Brooklyn, NY. In 2012 she moved to San Diego and works with an underserved population. She has worked hard to maintain the NAPBC accreditation at her affiliated hospital, Sharp Grossmont Hospital. She stresses the treatment of the “patient as a whole” and believes that outside of the exam room is where the difference is made in a patient’s long term outcome. She is also a wife and mother and loves to run. Thanks to our sponsor this week is Panacea Financial. Don’t forget to reach out to them for physician or medical student loans. #21: Heroes and Zeros! (Covidiots Part 2): “ A Doctales with Cocktails Episode”

Cheers to you if you are surviving the insanity wherever you live! The entire world is filled with the craziest rules, laws and regulations employed to “help” stop Covid-19. From forbidding sales of open toed shoes and short sleeve shirts  in South Africa to “banning pillows on your Brazilian...flight that is!  Canada has cracked down yet again with roadblocks and fines at  “Covid check points”. Other countries have employed tracking bracelets- que the Hunger Games music. The world is truly a crazy place. Soooooo….would you roll in cow dung or drink urine  if it meant you were protected from Covid? We are NOT kidding! Next we discuss the morality around vaccine incentives. You can get anything from free Krispy Kremes, free Uber rides, pot, handheld blenders, lottery tickets and ‘round the world plane tickets. State governments and Biden’s team are throwing carrots to the masses to reach their goal of 70% vaccination rates. Today we are drowning our disbelief with some tasty Peanut Butter Whiskey made by Skrewball Whiskey. Sipped over ice, this is a perfect aperitif that oozes with a nutty/caramel flavor. Plus we are toasting 29 years of wedded bliss. Thank you very much. #19: Sinking the Covid Needle or Not? (Part 2 with Dr. Peter McCullough)

#19: Sinking the Covid Needle or Not? (Part 2 with Dr. Peter McCullough)

Hang on to your seats, this is a long one! This week is a continuation from our podcast last week with Dr. Peter McCullough regarding   Covid-19!  Last week  we discussed  evidence based treatments for Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. This week is all about the vaccines, safety, adverse events and RISK!. Plus is there a bigger theme in all of this? It feels like we are fighting a war not against Covid, but against each other. With opposing sides in the public and medicine, to vaccinate or not to vaccinate;  who , what, when and why IS the question! We used to think asymptomatic spread was frequent and now we are learning otherwise. New evidence from published data available from tracking over  10 million people  in China shows there is a less than 1% spread  from asymptomatic people. The evidence seems to show we need to “Stop” testing asymptomatic well people! What we are doing in public health now seems wasteful, expensive. Why are we now still having to wear masks if we are vaccinated if it’s fully safe and effective as promoted? Next we look at extreme differences between the states handling the pandemic response from Michigan to Texas. Taking it further, look up north to the border to home and native land in Canada. The border is closed, Covid hotels are mandatory for noncitizens entering for ANY reason, with no regard to  their health, immunity  or current vaccination status. Draconian or Totalitarianism? You won’t believe it, but it’s TRUE!  Johns Hopkins published a booklet in 2017 referencing THE SPARS PROJECT. It was a “response” document and  plan to deal with the next pandemic , predicting even the exact genus and species that would hit the world in the near future. You can read how they had already mapped out how to handle, shut down and analyze a global crisis. Next we get into how the response to Covid-19 feels to be weaponized. Schools, businesses, churches, and countries shut down and often with massive fines for not adhering to authority. Is this really The GREAT RESET? We move on to discuss how the vaccine initially was promoted as a  “volunteer” basis to protect those at risk. Now we are seeing the implementation of  financial coercement, Universities mandating it, talk of vaccine passports, business threatening to fire employees who are unvaccinated. What!! While Dr. McCullough is pro- vaccine, he definitely believes that Covid-19 vaccination may not be appropriate for everybody. Covid survivors, potential childbearing women and pregnant women were ALL excluded from the trials, why would we expect them to blindly get the vaccine without any data? This is a risk too big to ask for many people and one that is naively endorsed by many. The CDC is very clear on their statement regarding who should get the vaccine : ask your doctor if the vaccine is right for you. They DON'T GET TO DECIDE! It is a discussion and decision between the physician and patient. No other vaccine( flu, pneumococcal, shingles, etc)  has been pushed this hard to the masses in the history of vaccines.  To date there are  3514 deaths associated with the vaccine: anaphylaxis , reactogenic deaths (from blood clots, high fevers, etc) and then thrombotic events that occur farther out in time. The numbers for vaccine related adverse events is over 40,000 per 100 million.  Something to consider: Is Johnson and Johnson the safest vaccine because it is only ONE vaccine? Finally: The proposition of vaccinating healthy low risk people to protect the population at large is fraught with moral implications. There WILL be college kids who die due to the rare event from vaccine induced causes .. from being  coerced into the vaccine because of pressure from the masses. How do we assess risk, benefit and sort through the data………….. What an honor to have this intelligent discussion and input from a world renowned and highly published physician.  Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology.  He also has a  Master of Public Health from University of Michigan School of Public Health. Dr. McCullough has broadly published on a range of topics in medicine with > 1000 publications and > 600 citations in the National Library of Medicine. His works have appeared in the New England Journal of Medicine, Journal of the American Medical Association, Lancet, British Medical Journal and other top-tier journals worldwide.  He is the editor-in-chief of Cardiorenal Medicine, Reviews in Cardiovascular Medicine, and senior associate editor of the American Journal of Cardiology.  He has appeared on news outlets and is passionate about spreading the word about treatment. #18: Dr. Peter McCullough on Early Covid-19 treatment

#18: Dr. Peter McCullough on Early Covid-19 treatment

This week is just the beginning of some AWESOME discussions around  Covid-19!  Get ready since we have an amazing guest and world renowned expert who is a leading expert on the subject of Covid-19 and  has the most citations in the National Library of Medicine.   Not only is he humble and gracious but very wise and  passionate about helping and treating our PATIENTS first!  Aside from being an expert, he TREATS Covid patients and  is also a Covid survivor.  Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology.  He also has a  Master of Public Health from University of Michigan School of Public Health. He cares for advanced patients with common medical problems including heart and kidney disease, lipid disorders, and diabetes. On November 19, 2020, Dr. McCullough testified in the US Senate Committee on Homeland Security and Governmental Affairs concerning early ambulatory treatment of high-risk patients with COVID-19. He has appeared on news outlets and is passionate about spreading the word about treatment. Today we discuss evidence based treatments of Covid-19 , best practices to avoid hospitalizations and death plus  what resources we have.  He believes it is immoral, unethical and bordering on illegal to advise NO TREATMENT  of outpatients who have Covid-19. The focus has been on PPE, lockdown and isolation as well as treating only hospitalized patients. Forgotten are the patients caught in the middle. We can’t just be passive doing nothing to prevent hospitalizations! It is therapeutic nihilism! While the FDA is a regulatory body, NIH is a research organization and the CDC is a public health organization- none of these organizations give guidelines and put the onus on physicians to figure out how to treat their patients. Remember physicians, we work for the patient! #17: The Doctor as Patient with Dr. Manisha Rayavarapu

#17: The Doctor as Patient with Dr. Manisha Rayavarapu

Today’s episode is the first in our series on “The Doctor as Patient”. So many times at work I have been shocked when patients find out I had the flu, had surgery or experienced a “real medical” problem. No we aren’t superheroes, and we DO share your pain.  When the doc becomes the patient, they often will experience the same struggles ,vulnerabilities and fears. This can be a very eye opening experience and impact how we see and treat our patients. This week we have the privilege to hear from Dr. Manisha Rayavarapu. She is a Family Medicine physician, diagnosed with Lupus during her 2nd year of residency training.  She describes how her faith along with her husband, have been the most important support on her journey. She shares her struggles with navigating the health system ,uncompassionate colleagues and administration, to family who may not understand how her illness affects her life. She has gone on to write a book on her experiences, struggles, fears and strength on coping with her ‘new normal’. Her memoir, “Butterflies, Boards and Blessings” is her way of helping educate and raise awareness about lupus.  She lives by her favorite quote : “life is 10% what happens to you and 90% how you react to it”! Our sponsor this week is Physician Financial Services. Reach out to Larry Keller for your disability insurance needs. #16: Doctales with Cocktails: Doctor Heal Thyself

Yippee it’s time for another Doctales with Cocktails! Cheers AND…..today we are diving into the beginning of our series on the “Doctor as Patient”.  We love to poke fun at ourselves, so we are sharing a few ‘memoirs” from our morbid past. Did you know Tim tried to be stoic beyond his common sense? Yep and he managed to land up with extended “staycation” at the hotel hospital, experiencing the pleasures of baseball and PCA pump full of morphine. We share our first childbirth experience. Come on who are we kidding- we all know it was actually MAY’s marathon experience!  Tim’s participation in the event was mostly a few minutes of “fun” and then having to split his brain between playing doctor or the dad card.  Best of all we are happy to dive into yet another flavor of our favorite Vodka- this week its 44 North Huckleberry Vodka. What a pleasant surprise! PS; they are NOT a sponsor but we sure would LOVE IT if they were! #15: A LIKE Story

It seems our fans are enamoured with our relationship and how we interact.  After 30 years together our marriage is thriving, the nagging has ceased. the socks on the floor don’t matter and we have solved the dishwasher argument. Most importantly we still LIKE each other. Guess what? Storybooks and Hollywood are fake! Relationships are not all saccharin perfection and you DON”T have to pull the plug or call it quits when things get tough. There honestly have been a few “I don’t love you any more moments” that we will share with you. Okay so maybe May did attempt to run away and abandon Tim and their baby. Join us for this honest conversation in what a successful marriage looks like and some of the struggles we have overcome. #14: We Have 4 F’s to Give

No we aren’t swearing mad! This week we are sharing our 4 F’s to success and what priorities need the most attention and balance in everyone’s life: Faith- yes everyone believes in something even if it’s nothing. How you see the world through your unique moral lens will shape everything you are. Family- love ‘em or hate ‘em, small or large, blood or your close tribe- everyone has relationships.  Humans need each other. Are you nurturing your clan? Finances- the dreaded “b” word ( budget), money or lack of it. Like politics or religion, everyone wants to avoid dealing with the finance topic. Even if you are  swimming in it you might not have your financial house in order. Fun- neglected by most. Can you guess it is a top priority for Tim? Are you living life to the fullest , neglecting yourself or  just screwing off irresponsibly? We also clear up all the Confusion and Congratulations over Episode 13! This episode is dedicated to Roxie, our producer. We had a wonderful 12 years with this yellow lab that was everyone’s friend. She made our hearts bigger, and took a piece of it with her this week when she passed. ...And by the way, if you need disability insurance, reach out to MR Insurance Consultants for any physician insurance related needs. #12: Doctales with Cocktails: Stupid “things” We’ve Done

This week is another Doctales with Cocktails and…..today we call BS on ourselves. We have done some really dumb stuff over the years and now we are laughing and sharing a FEW of the ridiculousness of our medical past. We have experienced exploding sebaceous cysts causing “something about May”, exploding speculum shrapnel and hydration on the legislature lawn. Tim has even been paid in whiskey for weekend vasectomies!  We confess we like our hillbilly medical experience which involves some vet medicine and NASCAR driving to emergencies. Biggest regrets: May’s 9 month stint as Tim’s nurse. Tim’s frosty faux pas “dicksicle”. We are tasting some Crater Lake goodness today- Cheers to Crater Lake Vodka & Cran for the win. We have touched on Fentanyl and the opioid epidemic. This week we interview Ben Westhoff , author of Fentanyl Inc. Westhoff has become the go-to expert on the subject of fentanyl and new drugs. His credits include consultation with top government officials, including the U.S. Senate and House of Representatives, the White House Office of National Drug Control Policy, and the U.S. State Department. He has been a guest on  The Joe Rogan Experience  and NPR Ben’s extensive research involved tapping into the history and stories of  various chemists, drug makers, drug dealers and criminals in the field. He went so far as  creating an online persona on the dark web and then managed to  infiltrate a Chinese drug lab while posing as a drug dealer! Say what??! #9: Fentanyl-The Personal Cost

#9: Fentanyl-The Personal Cost

This is a very somber, serious and sad story about the human element in our story about the Opioid crisis. We had the honor of interviewing our friends for this podcast so they could share their story. Just months ago, they lost their teenage son, Tanner. A scant amount of Fentanyl sprayed on a tablet of Tylenol took his life.  Everyone in the medical profession as well as the community at large will have their eyes wide opened to how this new crisis is overtaking our country.  As physicians, Tim and I have huge remorse for their family’s loss. But we also feel the burden of owning the problem as healthcare workers  who were pressured to treat the “5th vital sign”. #8: Doctales with Cocktails “Stuff Patients Say"

Woohoo, our second Doctales episode is full of cheeky fun and some of the stuff that makes us laugh. Mays doing a wine tasting while Tim is imbibing in the green fairy, stay tuned. So many funny things we hear patients say and do over the years! We keep this episode HIPPA compliant and share some of the crap we have seen and heard. We all know that patients are obsessed with taking their temperature, knowing their “normal temperature’ and upset if we challenge them on it. From “my kidneys hurt” to “my fever is normally 97 degrees” , we share some of the crazy fixations and verbiage. We know you all love foreign body stories...yep there's a story about misplaced garlic cloves and misuse of ‘moss kill” spray. Of course EVERYONE has a high pain threshold, and you all seem to think you need antibiotics for bronchitis.. these are a few of the myths we debunk ( and laugh at in the process) We never tire of getting complimented on how “young we look” but yes WE ARE old enough to be doctors. The begging for Zpack never ends….stay tuned for our new BS Free MD PACK!!  Cheers to you for being here and joining the fun! #7: The Flames of Hell are Fueled with Fentanyl

Move over Percocet, did you know that Fentanyl is the new kid in town? Things have gotten MUCH, MUCH worse with the emergence of Fentanyl and other synthetic psychotropic agents hitting the streets. This show continues on from where we left off on a previous episode discussing the rise of the Opioid epidemic. We discuss the exponential rise in fatalities,  international involvement,  general naivety to the problem and the difficulty in trying to curb this unprecedented and dangerous drug phenomenon.  As a reminder today’s sponsor is the Alpha Coaching Experience.  Act now to claim your spot in the spring enrollment before the doors close on February 22nd at midnight.              For more information, go to doctorpodcastnetwork.com/alpha LINKS from our show: Fentanyl Inc:                                 https://www.amazon.com/Fentanyl-Inc-Chemists-Creating-Deadliest/dp/0802127436 DEA Drug Facts:                              https://www.dea.gov/factsheets https://coloradosun.com/2021/02/04/colorado-fentanyl-overdoses/ Stay in the loop with us:                                                             Thanks for joining us! #6: Telehealth: The Good, The Bad, The Ugly

Like it or not, telehealth has fully arrived!  As Covid grappled the world, 2020  saw a dramatic rise in the implementation of telehealth services.  How has telehealth impacted you? Do you love it or hate it? We discuss the pros and cons for both physicians and patients, and share some laughs about some of our personal struggles with it. Patients: did you know there are some key vital signs and pre-visit things you can do to make your visit better?  For instance, as much as we like your newly minted eyebrows, we prefer to see your whole face!  We share a list of dos and don’ts that make your visit more successful. Physicians: patients mostly love the ease of access but also have some expectations from their doctor. We share some thoughts and also a few pointers and ways to get a good physical exam online. Hurray for home vitals!  Remember, when you need that contract reviewed to look up Contract Diagnostics at 1-888-574-5526 or check out https://doctorpodcastnetwork.com/contractdiagnostics Also, if you haven’t done so already, please subscribe to the show! #5: The Road to Hell is Paved with Percocet

#5: The Road to Hell is Paved with Percocet

In this episode, Tim and May discuss the single biggest mistake in the history of Medicine. The overprescribing of opioids started with a five sentence letter to the New England Journal of Medicine in 1980. This brief letter was quoted 608 times in Medical literature, espousing the safety of opiate prescribing. We dive into the history of opiates, opiate prescribing and drug abuse in general. We talk about how a simple misinterpretation of data caused the medical establishment, big pharma and government regulators to get it wrong and kill hundreds of thousands.  This peer pressure and group think produced three big lies: 1: We could  do it better with opioids “this time’ compared to a hundred years ago 2: Medicalization of opioids makes it “okay” 3: Treating acute and chronic pain has always been a challenge, therefore we have to make a  medical “solution”. And we also have a good laugh at the past and near present advertisements. From the printed ads used to peddle “mother’s little helper” cough syrup to the newer Oxycontin motto “One to start with and one to stay with”! Pain WILL be your “5th vital sign” after you hear how the opiate journey has unfolded. Meanwhile, our sponsor, On Time MD by Dr. Phil Boucher is offering 15% off their time management strategies course. #4: Doctales with Cocktails: The Covidiot Awards

#4: Doctales with Cocktails: The Covidiot Awards

In our first cocktails with doctales segment, we're discussing a certain topic that gives us a reason to drink… Covidiots. We're handing out awards for the dumbest Covidiots that made us laugh or just stunned us.  Everyone is trying to with what information we know. There has been good advice, bad advice, and wrong advice, so why not try to find some grace and humor out of this situation we’re in? We’re not just talking about people who call masks tyranny! Dr. Faucci warned early on in the pandemic that letting people wear masks is more likely to give them a false sense of security, and we’re baffled how some people seem to think this mask is a personal force-field! We also use this as a teaching opportunity to help encourage you all to use a little more common sense when wearing your mask.  So if you need to let off some steam with us, feel free to make a Vodka Gimlet and join us for some laughs.  By the way, you're here… and we love you.  If you have questions, reach out to us at [email protected] or find Tim and I on Facebook and IG. #3: Medicine... Was It All Worth It?

When you think about going into medicine, do you think about why you chose the field? Do you look back and wonder, “I thought medicine would be so much different than I know it to be now?”  To choose a subspecialty today, you have to have an immense amount of confidence in the unknown.  Tim talks about how this can become an undercurrent of self-doubt.  It takes a ton of digging deep and honing in on what you do well, knowing what you want to do with your life in a general sense, and recognizing the inevitable storms to weather. Do you really know what to expect?  Tim and I talk a lot about the becoming of a physician, along with the pros and the cons of gaining the white coat. Plus we will confess if we would do it all over again. Thanks for joining us! #2: Basement Blowups to Urgent Care Docs

#2: Basement Blowups to Urgent Care Docs

Looking back on our medical journey, Tim and I reflect on what brought us to Medicine. Tim comes from a medical family where his father was an independent GP, doing OB/anesthesia/call/everything. He understood pretty early on that medicine wouldn’t be an easy path.  I got the science bug early on but didn’t come from a family of doctors, or anyone with a college education. The pressure was real, though. I remember blowing up stuff in my parent’s basement and loving science in general. Plus, I just really enjoyed medicine on TV and so it kind of became my label. The needle was sunk into my family’s viens more than mine: send me straight to university into pre-med.   This episode is personal. It’s honest. And, it hopefully inspires some of you to pursue your dreams-- doctor and whatnot.  By the way, you are the reason we are here.  If you have questions, reach out to us at [email protected] or find Tim and I on Facebook and IG. #1: Should you sink the needle?

#1: Should you sink the needle?

You knew we would kickoff with Covid right?! Due to the timing of the pandemic and requests from friends, we knew we had to talk about COVID-19 vaccines. We are keeping it simple and talking about normal stuff like common misconceptions, how the vaccine works and what are their side effects? Who do we think should get the shot?  And, who should not.  What we know right now is that there are 5 types of vaccines that made it into phase 3 trials but 3 are really important to pay attention to. One called the mRNA (Messenger RNA) is absolutely fascinating. It’s incredible how it works as Tim explains in the show. So, listen… ok? Thanks for joining us!
