#43 Be Clear Why You're Here, The Key to Success in Nursing
In this episode, we’re getting real about something every nurse needs to reflect on.
I want you to ask yourself: Why are you here? Not just "here" as in your job, clocking in at the hospital or clinic, but what’s driving you? What made you get into nursing, and what keeps you from walking away, especially when it feels tough?
Look, I’ve been where you are. There have been times in my own career when I questioned if I was on the right path, or if I was just going through the motions. It wasn’t until I sat down and really dug into my "why" that things started to make sense again. And that’s exactly what we’re getting into today.
I’m going to get real with you about my own experiences—the good, the bad, and everything in between—because let’s be honest, nursing isn’t always a walk in the park. But when you understand your purpose, when you get crystal clear on why you do what you do, it becomes a lot easier to handle the tough days.
Key topics discussed in the episode:
Taking full ownership of your choices, your actions, and ultimately, your success
Dig deep into why you became a nurse in the first place.
Aligning your daily actions with your purpose
Being open to shifting your focus if your purpose changes over time.
So, I challenge you to take five minutes to think about your “why.” Write it down and keep it somewhere visible—your phone, your fridge, or even your bathroom mirror. And if you’re feeling brave, share it with a friend or colleague! Next week, we’ll dive deeper into deciding what you really want, so stay tuned. Remember, you’re not just doing a job; you’re building something bigger.

#40 Listener Q&A, A Serial Killer Nurse, and The Fight Against Alcoholism
First off, shoutout to all my listeners—y’all aren’t a huge crowd, but I love every single one of you. We’re keeping it real today, starting with a shoutout to @NoSaltBP.org, a nurse out of California who's got some fire questions.
This leads us into a discussion on the challenges nurses face across different states, especially when dealing with male physicians—both addressing their wrongdoings and how both male and female physicians can protect themselves from harm while ensuring patient safety and care quality.
Next, we dive into the chilling case of William George Davis, a serial killer sentenced to death for murdering four patients by injecting air into their arterial systems. We reflect on the gravity of such horrific acts within our profession and the pressing need for accountability and safety in healthcare.

#37 Special | Sedation in a COPD Patient | Listener's Question
Do you know how to handle COPD patients during sedation without screwing up?
Today’s episode is a special one, thanks to a great question from Debbie Hasty, a longtime friend and a badass PACU/ICU nurse. Debbie wanted some clarity on what to do when a COPD patient on home oxygen starts to desaturate.
First off, let’s discuss GOLD standard. GOLD stands for the Global Initiative for Chronic Obstructive Lung Disease Classification. GOLD 1 is mild, GOLD 2 is moderate, GOLD 3 is severe, and GOLD 4 is very severe. Understanding the severity of COPD (GOLD 1 to GOLD 4) is crucial because it dictates how we manage sedation and oxygen therapy. If you don’t get this, you’re screwing up from the start.
We’ll discuss the importance of getting COPD under control before you start sedating anyone. Remember, patients are notoriously non-compliant. You’ve got to make sure they’re actually sticking to their regimen, or you’re setting yourself up for a world of trouble.
We'll also dive into how to handle potential desaturation, ensuring you monitor closely without tipping the balance. It’s not just about following protocols; it’s about managing the fine line between under-oxygenating and over-oxygenating to prevent complications and provide the best care possible.
Key Takeaways:
Know Your GOLD Standards: Understand the severity of COPD (GOLD 1 to GOLD 4) and its implications for sedation.
Sedation Strategies: Use the right sedatives like midazolam or dexmedetomidine to minimize respiratory impact.
Monitor Closely: Keep a close eye on oxygen levels and patient status, adjusting treatment as needed without over-oxygenating.

#36 Patient Abandonment versus Refusal of an UnSafe Assignment
Do you know your rights when faced with Unsafe patient assignments?
In this episode, I’m diving deep into a critical issue that’s been making the rounds—patient abandonment versus refusing an unsafe assignment. If you’re a nurse and you’re not crystal clear on the difference, listen up.
I came across a TikTok video from a PICU nurse named Alex. She was about to be the only nurse on shift for a ridiculous number of patients—six, eight, maybe ten. And you know what she did? She said, "I’m not doing that shit," and walked out. Damn right, she did. If you think taking on that kind of load is okay, you need to check yourself. Alex’s story is a wake-up call for every nurse out there—don’t let anyone guilt you into compromising your license or your patients' safety.
This episode is all about arming you with the knowledge you need. We’re breaking down what patient abandonment really means and how it’s different from refusing an unsafe assignment. We’ll take a look at what states like California, Nebraska, Missouri, and Kentucky have to say about it. Spoiler alert: a lot of these states don’t know what the hell they’re doing either, but I’m here to help you navigate the bullshit.
So, whether you’re dealing with a supervisor who thinks they can push you around or you’re just trying to figure out where the line is between standing up for yourself and getting fired, this episode is for you. And let me be clear—getting fired is a hell of a lot better than losing your damn license.
Tune in, take notes, and then take action. It’s time to stop letting these so-called leaders throw us under the bus. They’ve got nursing licenses too, and they can damn well be reported just like you and me. So, listen up, bitches—it's time to transform nursing, one nurse at a time.

Throughout the episode, we underscore the critical importance of hourly rounds in patient care and highlight the severe consequences of failing to adhere to regulatory standards, such as the immediate jeopardy status now facing the hospital.
Key topics discussed:
Understanding Immediate Jeopardy: Immediate jeopardy is a serious term in healthcare regulation that signifies a situation where noncompliance with standards could lead to harm or even death to patients. It's crucial for healthcare facilities to address issues promptly to avoid severe penalties.
Importance of Hourly Rounds: The incident at Oregon State Hospital highlighted the critical need for hourly patient checks. Nurses must adhere to hospital policies and ensure regular, thorough checks to guarantee patient safety, even if it disrupts their sleep. Technology solutions should complement, not replace, human oversight.
Accountability and Leadership: The episode shed light on the role of leadership in ensuring patient safety. Administrators and managers must actively engage with staff, provide support, and address gaps in procedures promptly. It's essential to foster a culture of accountability and transparency to prevent similar incidents.

#24 Alaska Tribal Health Consortiums Are Legally Immune
Ever heard of the Alaska Native Tribal Health Consortium or Southeast Alaska Regional Health Consortium? Today, we dove deep into the complex world of tribal health consortiums and their legal implications, especially focusing on a recent Alaska State Supreme Court decision that affects how tribal health consortiums operate.
The court overturned a 20-year precedent, granting these consortiums a type of sovereign legal immunity that shields them from most civil lawsuits in state courts. This decision is monumental because it means that unless a consortium waives its immunity, they are largely protected from legal challenges.
We explore the implications of this ruling, discussing both the benefits and the challenges it presents. On one hand, this immunity can help consortiums avoid frivolous lawsuits, ensuring that funds are directed toward healthcare rather than legal fees. On the other hand, there are concerns about the lack of recourse for employees and contractors in cases of disputes or wrongful actions.
This topic is complex and can be polarizing. My goal is to inform and provoke thought, not to alienate any of our listeners. Whether you agree or disagree with the points raised, I hope this episode gives you a deeper understanding of tribal health consortiums and the legal nuances surrounding them.
Key topics discussed:
What tribal health consortiums are and examples like the Alaska Native Tribal Health Consortium (ANTHC).
The benefits and challenges faced by these consortiums
A detailed analysis of a landmark legal decision by the Alaska State Supreme Court and its implications on employees and legal suits.
The concept of "arm of the tribe" immunity and how it affects the legal landscape for tribal health organizations.

#23 Let's Take a Look at Heather Presdee's Case, Former Nurse Guilty of Triple Homicide
In this episode of the Transformation Nurse Academy podcast, we discuss the shocking case of Heather Pressdee, a former Pennsylvania nurse sentenced to life in prison for killing three patients by injecting excessive doses of insulin.
Learn about the details of her case, including her employment history at multiple healthcare facilities where she exhibited abusive behavior towards patients and staff.
Furthermore, we discuss the disturbing events surrounding Pressdee's actions and questions why her employers failed to report her behavior to the Board of Nursing, as mandated. This episode sheds light on the importance of recognizing and reporting abnormal behavior in healthcare settings to prevent harm to patients.
Key Points Discussed in the Episode:
1️⃣ Mandatory Reporting: One of the major issues highlighted in this case is the failure of multiple healthcare facilities to report Presidy's abusive behavior towards patients and staff. As healthcare professionals, we are mandated reporters and must speak up if we suspect any wrongdoing. It's crucial to hold employers accountable for not taking action to prevent harm.
2️⃣ Patient Safety: The tragic outcomes for the victims in this case underscore the importance of patient safety in healthcare settings. Presidy's actions resulted in the deaths of innocent individuals who trusted her to provide care. It serves as a stark reminder of the critical role we play in ensuring the well-being of our patients.
3️⃣ Accountability: The lack of accountability in allowing Presidy to move from one facility to another despite a history of abusive behavior is concerning. It raises questions about the oversight and monitoring of healthcare professionals within the system. We must advocate for stronger measures to prevent similar incidents from occurring in the future.
