Marley Medicure Manifesto to Dismantle the Traditional PBMs | Pharmacy Podcast Nation
"The only way to change the status-quo is to get off your ass and change it."
Dave Marley, Marley Drug:
It's not easy but it's possible to fight the 3 largest PBMs in your local community with your pharmacy services. Listen into this podcast & share with other pharmacy owners for several ideas on beating the PBMs in Community Pharmacy.
Thank you ApproRx & P.U.T.T. for sponsoring this podcast webinar.
Here's the back-drop information on Marley Drug & Medicure:
https://www.medicure.com/about
Work with Medicure today:
Neal Owens [email protected]
IN A RARE MOVE, A DRUG MANUFACTURER WILL BYPASS THE TRADITIONAL APPROACH AND PROVIDE ACCESS TO AFFORDABLE MEDICATIONS ACROSS THE UNITED STATES VIA MARLEY DRUG’S ESTABLISHED MAIL ORDER PHARMACY.
With licensing to deliver prescription drugs to all 50 states, Washington D.C. and Puerto Rico, and with an established vision to deliver low cost generic medications, Marley Drug Pharmacy was the natural choice for like-minded Canadian drug manufacturer, Medicure Inc., to further expand distribution of its products within the United States, included its branded statin, Zypitamag® (pitavastatin).
The acquisition allows Medicureto bypass wholesalers and pharmacy benefit managers (PBMs), in an effort to reduce consumer drug costs and provide patients across the U.S. easier access to Zypitamag®, which was extremely difficult for patients and physicians to access because of restrictions and high co-pays dictated by insurers and PBMs. Through Marley Drug Pharmacy, Medicurecan now deliver Zypitamag® to patients in every U.S. state without customers needing to use their insurance, or worry if the prescription will be covered. Medicurecan also provide Zypitamag® at a lower price overall, and a fraction of the cash price of a leading branded statin Livalo® (pitavastatin). As a manufacturer, Medicureintends to provide other branded medications using a similar approach, in addition to extremely low cost generic medications.
“It’s a perfect and responsible transition for Marley Drug,” said Dr. David Marley, CEO & President of Marley Drug. “Medicureshares our moral mission to cut drug costs for consumers. Since 2012 we have offered Marley’s Extended Supply Generic Drug Program, which was designed to battle the skyrocketing costs of name-brand drugs, and offer patients affordable alternatives, without increasing co-pays or limiting access to popular generics. We worked hard to acquire permits in every state. Now Medicurecan offer Zypitamag® from coast to coast at a fraction of the cost, without the hassle of PBMs and wholesalers.”
“Medicure and Marley Drug have the same vision. We want to deliver safe, efficacious and cost-effective medicines that benefit patients and healthcare providers. Our goal is to increase patient access to Zypitamag® while keeping the process and pricing as simple as possible. Through Marley Drug Pharmacy, we can now provide these benefits regardless of insurance or location,” said Dr. Albert Friesen, CEO of Medicure. “There will be no changes to the hours, location or to the great staff and customer service that Marley has built, and look to hire more staff as business grows. Dave Marley will remain a member of the leadership team, and we want Marley Drug to remain a trusted business within the community, and hopefully, this is a game-changing approach that gains attention nationwide.”

Opioid Madness Part Two | PAIN POD
March Madness inspired us, and our audience has waited long enough. What better time, than during a pandemic, to dive into “OPIOID MADNESS” and crown “The Greatest Opioid of All Time”!!!
In this episode of the “PAIN POD”, Mark “Pain Guy” Garofoli will continue the conversation from Opioid Madness Part 1, the opioid bracketology, or perhaps an “Opioidology”, but definitely a whole bunch of “Opioid Madness”. The battle of poppy versus poppy, and of course a bunch of man-made opioid opponents as well. Cool names and fancy formulations only get one so far, and we simply have no room for bias, thus pharmacology will rule as law of the land, to ultimately crown, “THE GREATEST OPIOID OF ALL TIME”.
The blood thirsty, constipating, yet definitely not sedating championship opioid brawl continues. One opioid, an opioid of all opioids, will remain standing atop the mountain of Mu, declaring its victory over all other opioids from all lands near and far. Which opioid will it be? The PAIN POD crowns the champ!

Tennessee Senator Shane Reeves | PBM Reform Podcast Series
Tennessee Gov. Bill Lee signed HB 786, a comprehensive PBM reform bill that strengthens existing fair pharmacy audit laws, limits PBM adjudication fees, and prohibits a PBM from reimbursing a pharmacy less than it reimburses a PBM-affiliated pharmacy.May 30, 2019
Tennessee SB650
Present law defines "pharmacy benefits manager" (PBM) to mean a person, business, or other entity and any wholly or partially owned subsidiary of the entity, that administers the medication and/or device portion of pharmacy benefits coverage provided by a health insurance plan, subject to certain exceptions.
Present law provides detailed requirements for the conduct of an audit of records of a pharmacist or pharmacy by a health insurance plan, a PBM, the state or its political subdivisions, or any other entity representing the same. One such audit requirement is that a clerical or recordkeeping error regarding a required document or record may not, in and of itself, constitute fraud; however, the claims may be subject to recoupment. This bill instead provides that a clerical or recordkeeping error identified during an audit is not prima facie evidence of fraud or intentional misrepresentation and must not be the basis of a recoupment unless the error results in an actual overpayment to the pharmacy or the wrong medication being dispensed to the patient. As under present law, a clerical error will not be subject to criminal penalties without proof of intent to commit fraud.
Shane Reeves is an American businessman, pharmacist and politician who is the State Senator for the 14th District of Tennessee, which is composed of Bedford County, Moore County, Lincoln County, Marshall County, and part of Rutherford County. As a seventh-generation Tennessean, Senator Shane Reeves has spent his life raising a family and opening businesses in middle Tennessee. He understands the importance of family, character, and conservative values to the people in this community.

NCPA 2020 Lifetime Achievement Award | Pharmacy Crossroads
Nasir Mahmood is a remarkable pharmacy owner. Indeed, so remarkable that the NCPA named him the winner of the 2020 Lifetime Achievement Award. And the list of honors and service he has provided to the profession and to his community justifies this high honor.
In this podcast Nasir and his son, Rehan, Mahmood share with Kneeland the PharmacyCrossRoads listeners several of the personal philosophy and business practices that allow Pine Plains Pharmacy, in Pine Plains, New York to be such an important part of this small community halfway between New York City and Albany, NY.
Nasir, who also earned an MBA shared his view that knowing people, calling them by name and finding ways for the pharmacy to stock the special or unique items they want is one critical success factor. While the pharmacy is small, just under 2000 sq. feet, he says they find ways to fit items into the pharmacy. Adding to that idea is a robust special-order program where he says find his willingness to orders items an important customer service.
Rehan tells how he worked with suppliers, the city’s fire department and created an appointment system that allowed them to provide this small town with a successful, and ongoing, COVID vaccination program.
One heartfelt aspect of Nasir’s philosophy is giving back, thus he has served President and Chairman of the Pharmacist Society of the State of New York, winner of the Bowl of Hygiea Award, serves on the State Board of Pharmacy and is active in both NCPA and APhA.

Cannabis Industry Election Victories | CRx Podcast
The cannabis industry won at the ballot box in the recent election, with four states passing laws to make cannabis legal for adults and two approving cannabis for medical purposes. These results moved the National Cannabis Industry Association (NCIA) closer to its goal of building a robust cannabis industry while supporting state laws and advocating for federal policy reforms.
On November 3, 2020, voters in five states made a definitive statement about cannabis use. Arizona, Montana, New Jersey, and South Dakota approved adult use of cannabis. South Dakota also approved a medical initiative, along with Mississippi, which approved comprehensive cannabis medical options.
The National Cannabis Industry Association (NCIA) has been working toward outcomes such as these since 2010. With 2,000 members nationwide, the organization is supporting the industry as cannabis becomes a thriving business and a real economic force. In recent years, NCIA has been a vocal advocate for social equity, working to promote expungement of criminal records of many people—largely from minority communities—who were unfairly incarcerated as part of the ill-conceived War on Drugs.
The results of the initiatives in these five states illustrate how far the industry has come. The people have spoken, and their voice was definitive.
“The margins were higher than we expected,” says Morgan Fox, spokesperson for NCIA. “In Arizona, there was a 60% margin, while the margin in New Jersey was 66%, 10 points higher than it was in California when it passed an adult use initiative in 2016. These results are not just political victories; they will likely have widespread ramifications. Importantly, it will be much more difficult for law enforcement to use cannabis as a weapon against minority populations. Instead, we will likely see more economic opportunities in the industry and fewer arrests overall.”
Although there were differences among the measures passed, those that supported legalization all set 21 as the legal age for use. In New Jersey, the law requires that the state set a regulatory structure, along with limits for possession and homegrown cannabis plants. Arizona established the number of plants allowed at six, while South Dakota limited cultivation to three. The law in Arizona also stated that the Department of Health Services must promote business ownership and industry participation among people from communities who have been harmed by former cannabis policies. Furthermore, a portion of cannabis tax revenue will be set aside for the newly created Justice Reinvestment Fund.
To date, almost 34% of Americans live in states with laws establishing cannabis as legal and regulated for adults. Four of the five states that recently passed laws legalizing cannabis have been responsible for roughly 60,000 cannabis arrests each year, mostly for simple possession. On the economic side, researchers estimate that the combined value of the cannabis markets in these five states will climb to more than $3.1 billion by 2025. In states where cannabis is now legal, Congressional representation will increase by 29 representatives and eight senators.
In sum, cannabis is now legal for adults in 15 states, as well as the District of Columbia and the territories of CNMI and Guam. What’s more, 36 states and several territories have comprehensive medical cannabis laws. Cannabis is now legal in some form in 47 states in the United States.

The Cancer Pain Conundrum | CRx Podcast
Opioids play a critical role in managing cancer-associated pain, but they’re highly addictive, and they don’t always provide sufficient pain relief for cancer patients. Many patients therefore turn to cannabis or cannabis-derived products to help manage their pain, using them either as standalone treatments or as an adjuvant therapy to opioids. But does research support cannabis use for this purpose? The evidence is mixed, and some studies suggest there may indeed be benefits for cancer pain—but there’s also reason for caution.
How Much Research Is There?
There’s a relatively strong body of research on cannabis for the treatment of chronic pain generally, according to Eloise Theisen, RN, MSN, president of the American Cannabis Nurses Association and co-founder of Radicle Health, a cannabis education and advocacy organization. However, much of this research has focused on neuropathic pain in multiple sclerosis and spinal cord injuries, and comparatively few studies have examined pain in the cancer context. Among the studies that exist for cancer, many have small sample sizes, and study design is also an issue. “We have animal studies and some clinical human studies,” says Theisen, but there’s “a lack of randomized controlled trials, which are considered to be the gold standard.”
A further complicating factor in the research on cannabis and cancer pain is that most of the existing studies on this topic have relied on products that do not match those cancer patients actually use in practice. According to Hance Clarke, MD, PhD, associate professor of anesthesia at the University of Toronto and author of a recent review article on cannabis and pain,1 the products that have been studied most extensively are pharmaceutical-grade cannabis isolates, such as nabiximols (an extract containing THC and CBD in a precise one-to-one ratio), nabilone (synthetic THC), and dronabinol (also synthetic THC). However, cancer patients outside of the research context are much more likely to use dispensary cannabis and non-pharmaceutical-grade plant-based extracts. “We’re kind of stuck without enough evidence on the products that people are currently using,” says Clarke. This is important, because patients may react quite differently to a dispensary product than to a pharmaceutical product, according to Clarke.

Eloise Theisen, MSN, RN, is a board certified Adult Geriatric Nurse Practitioner who specializes in cannabis therapy. For over 20 years, Eloise has worked primarily with cancer, dementia and chronic pain patients. In the last several years, Eloise has focused her efforts on cannabinoid therapies. Eloise was one of the first healthcare professionals to bring a clinical dosing regimen to the cannabis space. In 2019, Eloise was elected President of the American Cannabis Nurses Association and will serve through 2022. In 2019, Eloise co-authored Pain Free with CBD: Everything You Need to Know and Effectively Use Cannabidiol with Alice O’Leary Randall.

The Commitment to Evidence Based Content Development | Bernadette Mazurek Melnyk PhD
Fascinating discussion with the Ohio State University's Chief Wellness Officer & author of Evidence-based Competencies in Healthcare, Dr. Bernadette Melnyk PhD about evidence-based practice of medicine and raising the standards of content development.
Hospitals and healthcare systems around the world have attempted to build and sustain evidence-based practice (EBP) cultures and environments, yet they have struggled to clearly understand what EBP competency means. Implementing the Evidence-Based Practice (EBP) Competencies in Healthcare clears up that confusion and serves as a guide for clinicians, leaders, faculty, EBP mentors, and students in achieving EBP competency, facilitating evidence-based decision making in daily practice, and accelerating the speed at which research knowledge is translated into real-world settings to improve health outcomes and decrease healthcare costs.
We must develop content which is clinically focused in the same manner as practicing Evidence-based medicine.
Implementing the Evidence-based Practice (EBP) Competencies in HealthcareA Practical Guide to Improving Quality, Safety, and Outcomes By Bernadette Mazurek Melnyk, Lynn Gallagher-Ford, Ellen Fineout-Overholt
Bernadette Mazurek Melnyk, PhD, RN, CPNP/PMHNP, FAANP, FNAP, FAAN, is Associate Vice President for Health Promotion, University Chief Wellness Officer, and Professor and Dean of the College of Nursing at The Ohio State University (OSU) and Professor at OSU's College of Medicine.

Using Data to End Racial Health Inequality & Improve Health Equity | Transforming a Nation Series
Translational Health Disparities Research in a Data-Rich World
https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6356140/
People experiencing health disparities, researchers, program and policy staff, and leaders addressing health policies present a spectrum of opinion about the value of big data approaches. These differing opinions range from lack of trust, to acceptance, to enthusiastic endorsement. Researchers must be cognizant and respectful of these differences because building trust is a key aspect of building a broad coalition, which is needed for successful generalizability. Moreover, researchers have the responsibility to ensure their research does not cause harm to either individuals or communities.
How Can Big Data Help to Address Health Disparities?
https://datascience.cancer.gov/news-events/blog/how-can-big-data-help-address-health-disparities
Our country is in the midst of two crises. One is the threat of the COVID-19 pandemic. The other has more historic underpinnings but is now finally receiving greater recognition—the issue of racial inequality.
Today’s Guest:
Dr. Claire Exaus earned a B. S. Pharmacy degree in 1997 from Massachusetts College of Pharmacy. Got licensed in MA and worked at Cambridge Hospital for two months then moved to FL in June of that year and took a position of staff pharmacist at a very busy CVS pharmacy then called Eckerd Drugs. Dr. Claire Exaus quickly attracted the attention of the district manager as customers called and wrote the district office of her outstanding customer service. Dr. Claire got promoted to pharmacist trainer in May 1998 and spent a month in Richmond, Virginia assisting with Rite Aid conversion to Eckerd Drugs after acquisition.
In January 2001, Dr. Claire enrolled back in school into an MBA program but quickly realized that it wasn’t for her. Her goal was to be a clinical pharmacist and instead obtained her PharmD.
Four years later in January 2005, Dr. Claire was accepted into the PharmD program at Nova Southeastern University with a concentration in Managed Care and found her footing. In May 2010, Dr. Claire graduated with Honors with her PharmD while maintaining a full-time job and caring for her new baby.
In 2007, Claire left retail pharmacy and took a position at a federally-funded HIV clinic where she provided care and education to newly diagnosed HIV patients and assisted them in getting HIV medications through patient-assisted medication programs funded by drug manufacturers. Two years later, the clinic closed due to lack of funding.
In March 2009, Claire entered the world of hospital pharmacy at Baptist Hospital in Miami, FL. It was there at that institution that her clinical formation took shape and there Dr. Claire got to fulfill her goals of honing in her clinical acumen. Always ready to learn, she was assigned to different floors and was mentored by the more experienced pharmacists. She rotated through med-surg, pediatric, pediatric oncology, surgery, emergency medicine, critical care and oncology.
She was recognized for improving SCIP score rate after 3 months on the surgery floor. She demonstrated her leadership by leading various projects to improve medication safety and employee wellness with organizing fundraising for the annual heart walk. In 2011, Dr. Claire was introduced to pharmacy informatics by participating in a project to roll out new dispensing cabinets to a new surgery wing.
The rollout was an eye opener to a new world of potentials. Shortly thereafter, Dr. Claire took a position at Palmetto Hospital as a Clinical Informatics Pharmacist as the hospital was preparing to transition from Meditech system to Cerner.
Dr. Claire volunteered to train as a super user to train others to use the system. Dr. Claire trained physicians, nurses and pharmacy staff. Learning this new skill opened the door to go back to school and obtain a degree in Healthcare Informatics.
In August 2015, Dr. Claire left Florida and moved to Massachusetts and took the position of Lead Clinical Pharmacist at Healthcare Associates. She developed a pharmacist-led prior authorization program.
In June 2019, Dr. Claire transferred to the Cancer Center where she held the title of Clinical Coordinator Oncology System Management. Currently, Dr. Claire is pursuing a Masters in Regulatory Affairs with a concentration in Health Economic Outcomes Research.

The Challenges and Opportunities of Health System Pharmacists |Locked On Pharmacy
The Challenges and Opportunities of Health System Pharmacists
When the general public thinks of a pharmacist, they may think of the pharmacist behind a counter at a community pharmacy. But as we know, pharmacists work in all kinds of practice settings–and that includes thousands of pharmacists in hospitals and other health-system environments.
Their challenges are both similar to and different from community pharmacists.
Last year, APhA created a special council to discuss health-system pharmacy issues, and to advise the association on how to serve pharmacists in those settings.
Our guests are the council’s co-chairs: Steve Rough, from the nationwide pharmacy consulting firm Visante, and Noelle Chapman, from Advocate Aurora Health in Chicago.

Opioid Madness Pt. 1 | PAIN POD
March Madness is upon us, and what better time, than during a pandemic, to dive into “OPIOID MADNESS”!!!
In this episode of the “PAIN POD”, Mark “Pain Guy” Garofoli will talk us through an opioid bracketology, perhaps an “Opioidology”, but definitely a whole bunch of “Opioid Madness”, in order to pit one opioid against another. A battle of poppy versus poppy, and of course a bunch of man-made opioid opponents as well. Cool names and fancy formulations only get one so far, and we simply have no room for bias, thus pharmacology will rule as law of the land, to ultimately crown, “THE GREATEST OPIOID OF ALL TIME”.
We’ll start with a “Sweet 16” of opioids, mention a few bubble busting first four out, and navigate our way through the “Elite 8” and “Final 4” to culminate in a blood thirsty, constipating, yet definitely not sedating championship opioid brawl. One opioid, an opioid of all opioids, will remain standing atop the mountain of Mu, declaring its victory over all other opioids from all lands near and far. Just as Rocky Balboa declared his presence on top of the mountains of Russia, there will remain one opioid for all ages. Yet, all this opioid bracketology madness may not be the main point after all, tune in today to enjoy a brief journey through all things opioids, and learn more about what this madness is actually all about. Let the brackets begin!!!

Proven Ways to Market Your Pharmacy | Pharmacy Crossroads
Ask any pharmacy owner what the best form of advertising is and they’ll immediately reply, “Word of Mouth.” In this episode of PharmacyCrossRoads Bruce Kneeland shares several real live examples of what a pharmacy can do to dramatically increase the number of referrals patients provide. He also cautions that unless a pharmacy takes an active role in boosting word-of-mouth the natural tendency of people is to talk more about bad experience than good. The good news is, effectively handling things that go wrong can be one of the best ways to generate positive word of mouth.
Kneeland is interviewed by Robyn Amberg, a well-respected account manager for PrescribeWellness, a Tabula Rasa Healthcare company who introduces herself as Robyn Kneeland Amberg. She is Bruce’s oldest daughter and has garnered a well-deserved reputation as an industry expert in her own right.
The podcast covers a variety of topics intended to help pharmacy owners generate more positive word-of-mouth referrals. Kneeland and Amberg share ideas and examples of things they have seen pharmacies do. They also discuss the importance of getting pharmacy staff engaged in the mission of the pharmacy and talk about the importance of having a noble cause and the owner being able to create a culture of caring and commitment among the pharmacy employees.
Kneeland indicates he has digital copies of articles he has written and samples of customer referral card programs he is willing to share with listeners that request them via email: [email protected]

The Evolution of Education in Pharmacy | Pharmacy Podcast Nation
Today's special guest is Christopher M. Bland, Pharm.D., FCCP, FIDSA, BCPS Clinical Associate Professor at The University of Georgia College of Pharmacy.
As country after country imposed partial or total lockdowns in February and March, in 2020, the number of universities and colleges closing down their campus, interrupting face-to face activities, and switching to e-learning soared on a daily basis, eventually affecting more than 200 million students globally.
However, few of these institutions were well prepared for the sudden, disruptive move. A lot of scrambling, improvisation, and continuous adaptation have occurred as administrators, academics, and students struggle to implement online learning.
Impact on Students. The abrupt closure of campuses and the rapid switch to online education have disrupted students’ lives all over the world. It’s no surprise that students from underrepresented groups (low-income students, girls and women, members of minority groups, and students with special needs) have been hit especially hard—economically, emotionally, and in terms of digital deprivation. In poorer countries, students from disadvantaged groups face even greater difficulties.
