Improving the Primary Care Network DES service specification How to improve the PCN DES Welcome, please comment and I will try an answer SUBSCRIBE AND COMMENT Check out my original review where I went through the DES: here--https://youtu.be/K1qwyxt63f0 Declaration - I am not involved in the negotiations. I am Nottingham City East PCN Clinical director but these are my own thoughts. For some backstory in 2015 I wrote a further Blueprint for Primary care : https://egplearning.co.uk/a-further-blueprint-for-primary-care/ with a colleague as Jeremy Hunt launched the five year forward view. Some of these aims have been achieved (indemnity) but much can still be done.  How to improve the DES- I will talk firstly about general changes and the headline points. Then each specification in turn with my more innovative (controversial) ideas at the end.Ethos The release time of the specification has caused a lot of frustration. Even more so in the first webinar where little engagement was offered, just reading the spec. This led in part to one CD resigning live. NHSE doing an open consultation on the draft is a good thing. Feigning listening is not.  At points, the specification makes reference to evidence. In particular vanguard projects. However, it neglects to mention that vanguard projects have had significant higher levels of funding for each aspect and therefore converting this to specifications for networks to follow is a fallacy.  You cant use a recipe that feeds four people that costs £10 and say provide the same meal for £2 all the time - even with using foodbanks.  The original outline of the DES was to stabilise primary care. This draft does not do this but further destabilises it by utilising the additional roles above the capacity of work the can create and negates the fact networks will be part funding them. This was a poor way to show that NHSE/I was listening to the needs of general practice. Many comment the draft is intentionally too far to the right to allow any softening to be accepted as better than the original. This is a sceptics dream.  For this process to work- show General Practice that NHSE is truly listening. Much of this has been done amazingly by Dr Nikki Kanani and I do not envy her position. I am mindful of do not shoot the messenger being very apt here.  Lets move on. The following is a combination of my views and those from the various forums I engage in.  Trust  General practice. If I had control I would keep existing structures that could help foster better collaborative working and simply offer the resources to networks to use for their local area.  General Practice has proved to be the most cost-effective method of delivering healthcare currently doing 90% of the NHS workload for less than 9% of the funding. Trust the networks to deliver primary care with the simple metric of improving the health of the network population.  Ideally to the global sum but is not then to the networks directly to use as they seem fit to provide roles or services for better patient care along them 5 categories as best for their population.  However, we do not live in a world of trust despite GPs consistently being the most trusted role in the country.  So if working towards the confines of adjusting the existing DES and its specification let us have a look.  Worth looking at the Berkshire, Buckinghamshire and Oxfordshire LMC document link:

Primary Care Network DES specification review 2019 My review of the 2019 Primary Care Network Service Specification- the good, the bad and the confusing.  This is the live recorded review of the draft primary care network service specification.To see the document in full use this link.PCN Clinical Director resources playlist:General Practitioner resources playlist:Make sure you leave your own feedback via the survey here (see bottom of page), or send an email to [email protected] is the draft version of my notes.  Welcome My perspective on the primary care network service specs Cover the good, the bad and the confusing. Check out my PCN resources and General practitioner playlist (in show notes)Save you and your patients time with tech enhancing your primary care and learning Why I am talking about this - am a GP also PCN director of NCE - 66k patients in a deprived area of Nottingham.  Firstly the timing of the documents Release of specs day before Xmas eve- while aware impacted by the election, purdah and process, it speaks to the perception that when PCN CDs work is irrelevant to NHS England.  However, I commend Nikki Kanani’s engagement on SoMe and the will to gain feedback before making the spec official,  as well as engagement events beforehand (twitter chat and webinars) So emphasis that this is draft specifications and can be changed. So let us take a look….. The document starts by talking about the investment being offered. While there is a significant increase in funding being offered, it is important to note the UK still invests less than most other western countries especially in the G8 at about $1000 less per person.  It also mentions a 5-year contract. I would like to mention that it is not really a 5y contract but a 5y agreement of the funds. If it was a contract the terms would be clear from the start. Instead, we are having ongoing annual negotiations of the requirements - which is no different to what we have been exposed to in primary care.  It then identifies seven asks or as they have been locally called the significant seven of which five are to start from April 2020 and another two from April 2021 Structured Medication Reviews and Optimisation • Enhanced Health in Care Homes (jointly with community services providers) • Anticipatory Care (jointly with community services providers) • Personalised Care; and • Supporting Early Cancer Diagnosis1.8 When will we have clarity on the process and use of the Network Dashboard? 1.11 Funding is not allocated directly for the delivery of the service specifications - let that sink in.  1.12 Extra capacity from the new roles in 20/21 - much of this was promised to stabilise practices not ask them to do more and does not allow for the extra 30% investment which is not supported by the existing pots.  1.16 funding for community service providers commented - limited

How to register with your local GP (2019)

How to register with your local GP To obtain healthcare from an NHS GP (General Practitioner) you need to be registered with a practice. In this post learn how you can register with your local GP.  Registering with your local GP surgery is an easy process, but there are some considerations to make. Listen below to hear how, or watch the video at the end of this post to see more. In summary: By paper: You need a GMS 1 form. You can obtain this from your practice, or download from the nhs.uk site here. An example is below: GMS 1 form front When completing the form, complete all relevant sections on the first page: Your details - needed to register youDetails of your previous practice or address- to track your records quicklyIf you are eligible for veteran care as part of the armed forces.If you are registering your child- if you need ongoing child health surveillance care (under 5's).If you need the use of dispensing service ( where the practice provides your physical prescription instead of a local pharmacy). This is more common for rural practices.Opting in/out of organ and blood donation services. The top of the back of the GMS 1 form is completed by the practice (not the patient). The final section on the back is relevant if you are an ordinary resident (normally living abroad or visiting England) to confirm the level of care you may receive. Other ways to fill the form are to access it via your practice website. Reasons a practice may not register you: Sometimes a practice may not be able to register you. This would be if : the practice list is closed to new patientsif you are outside of the registered practice areaif you have previously been removed from a practice list (certain situations and rare). You may be asked to provide photographic ID or proof of address. This is not compulsory but recommended as it can help speed up the registration process. This includes: passportdriving licenceresidence permitARC cardutility bill Also, providing a copy of your repeat medications can make a new registration easier. The GMS 1 form is different from the temporary registration form and provides a different service. To find which practices you can register with ask local residents, community leaders, housing support teams or use the nhs.uk website here. NHS App You can register via the NHS app. This is the gateway to health services and enables you to register with a practice quickly via your smartphone device. The NHS app also allows you to: book an appointment at your local practiceorder your regular (repeat) medicationslook at your medical recordsuse a symptom checkersearch health condition information from the nhs.uk websiteconfirm your organ and blood donation choices Further functionality is due in the next few years and some areas may have extra services. Other apps Other companies can provide you with health care via an app system. Social Media Guidance for Doctors and clinicians: pre-SCRIBE Are you aware of the social media guidance that exists to keep doctors, clinicians and patients safe? In this post, I show you various forms of formal social media guidance, why these are needed and explore my own easy to use version called pre-SCRIBE. Following recent articles in magazines like STAT that have commented why doctors shouldn’t use social media, other reply articles by the organisation SoMeDocs and SELF have shown why doctors and clinicians should. Whether hard or soft, in terms of cost this may vary from bargain basement rucksack from your local shop to designer medical or retail bags including maternity bags. In reality what you truly need is simply a comfortable bag to carry, with several pockets, somewhere to keep paperwork and ideally a lock on one compartment if you plan on carrying FP10 prescriptions and/or meds. https://youtu.be/UBGMDYEzMYo The bags I mention and more: The Merlin Morgan bag - was my first doctor's bag. Really large, robust and more pockets than a magician (hence the name I guess). I now use it as my camera bag. Alternatives are the Gima utility bag with a packet which a colleague has used for a while. The extra packet is useful for keeping medications and small items in. Alternately an effective camera bag can be a great choice as a doctor's visiting bag and on several forums, these are often the preferred options my many. My first Hi-Tec rucksack visit bag was ok but my current Hi-Tec visiting bag is much better. It has lasted me about 4 years now, is large enough to carry everything I need but still small enough to be portable and functional. The material could be better padded and insulated, an issue in cold weather which can affect equipment working like thermometers. Trainee bag- Duragadget This is the doctors bag we have for our trainees. It is similar to the Hi-Tec bag above but solves the insulation issue. It is also waterproof. Harder bags like pilot chest are preferred by many given they are more robust and business like. Or makeup chests can be a great option if you want a really solid bag that you can also sit on. the multiple compartments make this really useful. Kipling bags: and Mulberry bags are good fashion bags that can be effective as a visit bag. NOt my particular preference but often a good choice and ones several colleagues have used. IYASU is a company that is looking to make visit bags specifically designed for doctors in the UK. One tool to consider is Clockify which can easily be used to track your time. See our video which explains this tool and why it might be the best one for use in general practice. Text expanders and autoconsulations Each system in primary care has its own method of helping you document more information quickly. EMIS has shortcodes, Systm One has autoconsultations. If you want a tool that sits outside of the clinical system, consider a text expander like in the eGPlearning support pack. Things to consider is hot to personalise these tools and the probity of use. Automation and protocols Each clinical system has ways to help you automate the work you do. SystmOne has autoconsultations and protocols, EMIS has macros, and I am sure other systems have similar. Using these tools can significantly save you time. As SystmOne users, Andy and I use autoconsultations for streamlining work like our anticipatory medication prescriptions for end of life care. Workflow and remote working How you manage workflow in your practice will drastically affect your workload. This is a must see video if you are about to become a newly qualified GP.
