Stigma Surrounding Substance Use Disorder in Anesthesia Providers: Are We Part of the Problem?

Stigma Surrounding Substance Use Disorder in Anesthesia Providers: Are We Part of the Problem?

Substance Use Disorder (SUD) impacts all types of people but trying to confront addiction is never easy. Today we’ll try to unravel the stigma surrounding SUD among anesthesia providers with Amy Yerdon, DNP, MNA, CRNA, CNE. Our discussion illuminates the personal and professional challenges that arise when confronting substance abuse within our ranks. We tackle the complexities of recognizing signs of diversion and the imperative actions that follow, while sharing the poignant emotional toll it takes on everyone involved.   Here’s some of what we discuss in this episode: Her perceptions about people with SUD and how that’s changed as she’s learned more about the disease. Stories that highlight how much more common this is than many people want to believe. Understanding the pharmacology and physiology of substance use disorder may help decrease our negative perceptions. Understanding the occupational and personal factors making anesthesia providers more likely to fall into SUD may help change negative perc Why anesthesia providers should look out for one another and communicate when they see something.   About our guest: https://scholars.uab.edu/6674-amy-yerdon   Visit us online: https://beyondthemaskpodcast.com/   Get the CE Certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf   Help us grow by leaving a review: https://podcasts.apple.com/us/podcast/beyond-the-mask-innovation-opportunities-for-crnas/id1440309246   Donate to Our Heart Your Hands here: https://www.ourheartsyourhands.org/donate

From Standards to Testimony: The Anatomy of Medical Legal Review

From Standards to Testimony: The Anatomy of Medical Legal Review

Today we're going to dive into an interesting topic: some medical legal considerations. And we’ll talk a little bit about being an expert witness. We'll cover anesthesia standards created and published by the American Association of Nurse Anesthesiology, and share some of our own experiences acting as expert witnesses in medical legal complaints filed on behalf of patients. Perhaps just as important though, we're going to talk about ways that you can keep yourself out of hot water and keep your patients safe. It's important that as providers we periodically review the standards by which we're expected to practice. Both for the benefit of our patients and to make sure that we're practicing at the highest levels of anesthesia care every single day.   Here’s some of what we discuss in this episode: Reviewing the Standards for Nursing Practice from the Professional Practice Manual. The most common problems that I have seen that have resulted in litigation fall into four main categories. Who evaluates whether or not the standard of care has been satisfied?        The difference between a deposition and trial testimony and how it impacts you.   Visit us online and get show resources here: https://beyondthemaskpodcast.com/   Get the CE Certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf   Help us grow by leaving a review: https://podcasts.apple.com/us/podcast/beyond-the-mask-innovation-opportunities-for-crnas/id1440309246

Lessons Learned from a CRNA Campaign for PA State Senate

Lessons Learned from a CRNA Campaign for PA State Senate

We learned years ago that failing doesn’t mean you are a loser, it means you are a learner. Today’s guest unsuccessfully ran for office but that experience provided plenty of opportunities to learn and grow. Matt McCullough, CRNA shares those lessons learned during his campaign for the Pennsylvania State Senate in 2022 and why he has no regrets about pursuing change in his state.    Here’s some of what we discuss in this episode: How a conversation around a table at home made him pursue a spot in the PA State Senate. Why he feels nurses are the perfect candidate to serve in politics. What are the steps you have to take to get a campaign started? Why he wishes he would have started sooner to build relationships and meet more people. Understanding the demographics of your district before you run. Crisis Management on Demand

Crisis Management on Demand

This is a super fun episode where we welcome Mark Gabot, DNP, CRNA, FAANA on the show as a cohost. Mark and Jeremy presented at the 2023 AANA Annual Congress in Seattle and recorded this episode at the same time. Jeremy and Mark talk about how we think during a crisis, the unconscious biases that exist and they focus on a cognitive template for crisis management that can help formulate a working diagnosis of the perioperative problem, and then zero in on appropriate management. They also have some fun with some guests in the audience and invite them up on stage to talk more about their areas of expertise. Jeremy and Mark first invite one of the authors of the published Cognitive Template article (Lisa Osborne-Smith, PhD, CRNA, FAANA) up on stage to talk more about the utilization of the template. They next invite the Cormac O’Sullivan, PhD, CRNA, ARBP, FAANA to talk about simulated crises at the AANA Annual Congress. Finally, they invite both Sharon Pearce, DNP, CRNA, FAANA and Jeremy Stanley, EA, CFP®, AIF® up on stage to talk about the origins of the Beyond the Mask Podcast.   Here are some of the things we’ll discuss in this episode: How we develop our own mental model of what to follow in terms of treating a patient. The two modes of thinking when we make rapid decisions. The research on cognitive errors and biases. Background on the cognitive template article from Oregon Health & Science University. Simulation training with Dr. Cormac O’Sullivan. Fighting for Inclusivity: How a CRNA Changed Her Community with Maureen McCartney Anderson

Fighting for Inclusivity: How a CRNA Changed Her Community with Maureen McCartney Anderson

Our guest today has quite the story to tell about advocacy but it’s not the same advocacy we normally discuss on the show. Maureen McCartney Anderson, CRNA has been in-volved with her state association in New Jersey for a number of years and was on the path of advocating for professional rights, but that changed dramatically when her baby was born with handicaps. Now she’s pouring much of her energy into advocating for the special needs children’s population and making a difference in her community with the help of other CRNAs. She realized quickly that some children were being left behind at the playground because they lacked inclusive options for children and adults of all abilities. So she took on the task of advocating for an inclusive playground in her local community of Mansquan, New Jersey. In doing so, they developed a charity called Friends of Manasquan Parks and Recreation to help expand the mission and vision they had for recreation initiatives.  Things really took off when the group ‘CRNA Moms’ got involved and the two-year project was officially completed in December 2022. Today we’ll learn more about Maureen’s advocacy efforts and how the CRNA community rallied around a wonderful cause.   Here are some of the things you’ll learn on this show: The story of how this mission became a top priority for her after her son were born. How she’s helped educate people on the need for inclusivity. The back-and-forth efforts they went through on design and why they cared so much about making it fun for everyone. The incredible support she received from the CRNA community after reaching out to the CRNA Moms Facebook group. The different fundraising efforts they took on to raise money for this $900,000 project. Discovering the Healing Power of Battlefield Acupuncture with Angel Jaap, CRNA

Discovering the Healing Power of Battlefield Acupuncture with Angel Jaap, CRNA

Battlefield Acupuncture (BFA) is rapidly gaining recognition for its effectiveness in alleviating acute and chronic pain, as well as anxiety. When we heard Angel Jaap, CRNA speak about this at the AANA Spring Symposium, we wanted to bring her on to learn more about this alternative therapy. You’ll understand why she wants more nurses to learn this protocol after you hear some of the applications and success stories she shares today.   For those that aren’t familiar with BFA, patients undergo insertion of five auricular semi-permanent (ASP) needles to the following traditional ear acupuncture points bilaterally: Cingulate Gyrus, Thalamus, Omega 2, Point Zero and Shenmen. Angel learned about this over the past few years and has instituted a Pilot program through Whole Health within VISN 4 which allows RNs to perform BFA for veterans thereby increasing veterans access to this safe, effective pain management therapy.   You’ll learn more about that and have a much better understanding of BFA after listening to our conversation during our trip to Hershey, Pennsylvania.   Here are some of the things you’ll learn on this show: How therapeutic touch works and why it makes you feel the way it does. How BFA was developed and why the VA began incorporating this into patient care. What does a day at work look like for Angel? Angel shares a wonderful success story about treating a veteran and PTSD. The smaller needles used in BFA have been effective for people scared of needles.   Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf

PCN Allergy…Why Not Give Ancef?

Caring for surgical patients who have an associated penicillin allergy is relatively common. Should we avoid cefazolin (Ancef) in these patients? In this episode, Jeremy and Sass look at current and past evidence to uncover the truth about the relationship between penicillin and Ancef. This episode will get you thinking about the antibiotics you administer in your anesthesia practice.   Here are some of the things you’ll learn on this show: An estimated 8-10% of patients have an allergy to penicillin and where did this projection come from? Why many people might think they have a penicillin allergy but test negative. Why are we concerned that medications might cause allergies? What the information and studies tell us. Three cases where a patient had a documented penicillin allergy and received Ancef. The efficacy of the other antibiotics. What a liability study discovered. Our key takeaways from the evidence we have.   About our hosts: https://kpatprogram.org/about-the-school/faculty.html  Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf

Video-Laryngoscopy Including Flexible Intubation (VIFI)

Have you ever managed an airway with both a video-laryngoscope and a flexible intubating (fiberoptic) scope? Wait…what??? Yep, in this episode Jeremy and Sass talk about an evolving airway management technique that utilizes both devices to help secure difficult airways. They call it VIFI!   Here are some of the things you’ll learn on this show: What is VIFI and how does it work? What are the advantages to using this technique? Case reports of airway obstruction and airway closure. A case study of successful airway management using video laryngoscope. How do you perform this process?   About our hosts: https://kpatprogram.org/about-the-school/faculty.html  Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf

Doctoral Project Review – Peripheral Nerve Catheter Insertion

Doctoral Project Review – Peripheral Nerve Catheter Insertion

Today we welcome in two more guests as part of our doctoral series, and we're looking forward to talking with Annie Brademeyer, CRNA and Amanda Janssen, CRNA about their recent study:  “Does inserting peripheral nerve catheters over the needle decrease leaking from its insertion site?” Join us as we review their work and find out what they learned about a surprisingly controversial topic.   Here are some of the things you’ll learn on this show: What are some of the problems that are encountered by clinicians who place these peripheral nerve catheters? The different insertion method of over the needle and through it, and what impact that has on leaking. The challenges they faced in doing this study. What they found out about pain scores. Why they need more randomized controls to get more clarity on the topic. The need for these continuous peripheral nerve catheters moving forward.   Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf

Perioperative Cannabis Implications for Anesthesia Providers with Dr. Daniel King

Perioperative Cannabis Implications for Anesthesia Providers with Dr. Daniel King

The rate of cannabis use has exploded exponentially in the past 10 years and the widespread consumption demands us to be knowledgeable on the topic. Dr. Daniel King, CRNA, has become a dedicated subject matter expert in cannabis implications for anesthesia patients after taking an interest in the issues he was observing in his day-to-day practice. Today he’ll help educate us on the subject, the different risk factors, and what he’s learned about providing anesthesia for these patients.   Here are some of the things you’ll learn on this show: The legal status of cannabis today and has it’s evolved rapidly over the last few decades. The lack of education on this subject despite the fact that cannabis use has overtaken tobacco use. The different ways patients are using and consuming cannabis. What we know about THC concentrations and anesthesia requirements. The cardiac, respiratory and other risk factors for patients who use cannabis.   Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf

Demystifying Awake Intubation

Demystifying Awake Intubation

Awake tracheal intubation is a procedure that many of us rarely perform. In this episode, Jeremy and Sass focus on the indications for awake intubation.   They talk about the 2022 ASA Difficult Airway Algorithm portion that focuses on awake intubation, as well as the process and considerations for performing it. Take a listen and help demystify this infrequent (but important) airway management technique.   Also, if you are planning to attend the AANA Annual Congress in Seattle on August 20, consider stopping by Jeremy’s lecture on “Topical Thunder: An Awake Intubation Technique That Works” and see him use the technique on stage to intubate himself.   Here are some of the things you’ll learn on this show: What is an awake intubation and why would you use this procedure? The overall incidence of awake intubation is actually pretty low. The history of the ASA algorithm and what it says along with the other algorithms out there. What was updated in the 2022 ASA difficult airway algorithm. So when should this procedure actually be considered? The techniques used during an awake intubation.

Lessons Learned in the State Legislature with Taylor Rehfeldt, CRNA, and Sydney Davis, CRNA

We’re back with another great interview from our trip to the mid-year assembly in Washington D.C. as Sharon and Tracy Castleman, DNP, CRNA, APN-A, FAANA got to sit down with Taylor Rehfeldt, CRNA, and Sydney Davis, CRNA. You might remember them from episode 111, and we have been excited to catch back up with these two state representatives to find out what they’ve learned about advocacy and politics by being on the other side of the table.    Here are some of the things you’ll learn on this show: How Taylor ended up in this new role as assistant Majority Leader in the House of Representatives. What pushed them to run after they both got exposed to the legislature through involvement with the state association and the leadership there. The big differences they’ve found from being on the other side of the advocacy push, especially as it relates to campaign finances. You can make a difference in someone’s campaign by donating your time or talents. Tips for helping CRNAs get bills passed after they’ve seen how much time and effort goes into getting a bill passed. Both women share a few of the lessons they’ve learned over these past 2+ years in office.   Check out their first appearance on the podcast here: https://beyondthemaskpodcast.com/ep-111-finally-two-south-dakota-crnas-elected-to-state-legislatures/2769/    Support Team Emma Kate Brekken: https://grouprev.com/HALOSWALK2023-grant-brekken  About our guest: https://www.taylorrehfeldt.com/ https://www.votesydneydavis.com/ Visit us online: http://beyondthemaskpodcast.com  Get the CE certificate here: https://beyondthemaskpodcast.com/wp-content/uploads/2020/04/Beyond-the-Mask-CE-Cert-FILLABLE.pdf
